Visiting Us for the First Time

We welcome all new patients.  We offer both appointments or Walk-In Services.  It is our goal to make your visits as convenient as possible.  Please arrive at least fifteen minutes before your scheduled appointment with the necessary information.  Please visit the Patient Forms section to download the correct forms for your visit.

First Visit

Remember To Bring:

  1. Your insurance cards

  2. All current prescriptions and/or over the counter medications

  3. Patient information about your medical and surgical history.  Any recent or previous medical records sent to our office prior to your appointment.

Confirmation & Cancellation 

You will receive a phone call appointment reminder, letting you know of your upcoming appointment.  If unable to keep a scheduled appointment please call our office 24 hours in advance.

Here you will find all the forms necessary to bring with you for your first visit. These can be downloaded, printed and filled out. Completing these forms before your first visit can help your appointment proceed in a timely manner.

We are dedicated to providing the best possible care for you, and we want you to completely understand our financial policies.


  • Payment is due at the time of service unless arrangements have been made in advance by your carrier.  We accept Visa and Mastercard


  • Keep in mind that your insurance policy is basically a contract between you and you insurance company.  As a service to you, we will file your insurance claim if you assign the benefits to the doctor.  In other words if you agree to have your insurance company pay the doctor directly.  If your insurance company does not pay the practice within a reasonable period, we will have to look to you for payment.


  • We have made prior arrangements with many insurance companies and other health plans to accept an assignment of benefits.  We will bill them, and you are required to pay a co-payment at the time of your visit.


  • Not all insurance plans cover all services.  In the event your insurance plan determines not to cover a service you will be responsible for the complete charge.  Payment is due upon receipt of a statement from our office.


  • We offer a reduced rate for an unisured patient.  Check the Patient Forms section for a Sliding Scale Application.
